    Guidelines for Processing Business Permits and Licenses

    Requirements

    1. Business Application Form
    2. Lease Contract between the Lessor and the Lessee. If leased, attach Mayor's Permit registration of the building owner/landlord as a Real Estate Lessor. If owned, transfer Certificate of Title (TCT) or Tax Declaration (photocopy).
    3. Business Name Registration with the Department of Trade and Industry (for sole proprietorship/Single Proprietorship)/Certificate of Registration from Securities and Exchange Commission (SEC) or Cooperative Development Authority (CDA).
    4. Articles of Incorporation or Partnership (approved by S.E.C.)/ Article of Cooperation
    5. Barangay Clearance (business located)
    6. Market Clearance, if applicable
    7. Real Property Tax Clearance
    8. Locational Clearance
    9. Special Power of Attorney with photocopy of ID of the authorized to transact and of the taxpayer/applicant
    10. Copy of Occupancy Permit
    11. Other Requirements required by law such as:
      1. Certificate of Exemption, if applicable;
      2. Certificate of Good Standing and Good Status from Cooperative Development Authority (CDA) in the case of cooperatives
      3. Health & Sanitary Certificate
      4. Philippine Coconut Authority (PCA) Clearance (coconut lumber only)
      5. Department of Trade and Industry (DTI) Registration (establishments with trade names).
      6. DENR Certification (sawmills, lumber retailers, quarry).
      7. Insurance Commission Permit/Authority (insurance companies only) or Authority to operate an insurance Branch from the main office (insurance branches only).
      8. Central Bank Authority/Certification (banks & pawnshops only)
      9. Department of Labor and Employment (DOLE) Authority (recruitment agencies only) of Authority to operate a recruitment branch from the main office (branch recruitment agencies only).
      10. Clearance from the Optical Media Board for videograms
      11. BFAD license (for pharmacy / drugstore only)
      12. POEA license (for employment agencies)(overseas)
      13. CHED (permit for Review Centers/Tertiary Learning Institutions only )
      14. TESDA( Permit Vocational, technical and skills training centers only)
      15. DepEd Permit ( for Elementary and Secondary learning institutions only)
      16. PNP permit Security Agency (PSCUCIA National License)
      17. DOE License (for Gasoline / LPG Dealers and retailers only)
      18. DOH license ( Hospital / Clinical Labs / rehabilitation Center/Clinics only)
      19. DENR license ( Mining Industry only)
      20. Such other permit or clearances from regulatory agencies not enumerated herein but required by law.
      21. Other pertinent information and data as may be required.

    If approved -Licensing Personnel will encode profile into the BPLS System, If disapproved - all documents will be return to the clients and inform him regarding the status of his/her application 10 minutes Cynthia Rojo

    2 Pay- Present encoded application form for assessment and payment Assess tax due, permit and regulatory fees. Issues Official Receipt and received required payment. 15 minutes City Treasurer’s Office Depends upon the assessment of CTO Official Receipt/ Real Property Tax Clearance
    Backroom Activity : BFP will issue official receipt and FSI Certification 15 minutes Bureau of Fire Depends upon the assessment of BFP Fire Safety Inspection Certificate/Official Receipt
    3 Submit the paid/ encoded application and official receipt with complete attachments back to the Licensing Section and Claim approved Business permit with business plate and Sticker after signing the registry book Check and receive the paid/encoded application form and all the requirements with photocopy.     1. Previous years Mayor’s Permit/License
    2. Official receipts of payments — 1st to 4th quarters for the preceding year
    3. Special Power of Attorney (SPA) with photocopy of ID of the person authorized to transact and of the taxpayer/applicant
    4. Community Tax Certificate
    5. Income Tax Return and Financial Statements for the preceding year which were filed with the BIR on the current year. For consolidated FS, attach breakdown of gross sales/receipts from other City/Municipality. For Fiscal year FS, attach breakdown of gross sales/receipts from January to December.
    6. Barangay Clearance (business located)
    7. Market Clearance, if applicable
    8. Assessor’s Certification or Real Property Tax Clearance
    9. Other pertinent information and data as maybe required
